Intensive Support Services (ISS) provides mentoring, crisis intervention, individual counseling, and evidence-based group programs to Sawyer County youth and their families. The Care Coordinator provides one-on-one, family, and group services to help clients work toward goals and provide support and resources. This position meets with clients primarily in the community and assists with transportation as needed.
Responsibilities:
- Review referrals and collaborate with referral sources to discuss information to guide service planning and approaches
- Schedule and complete intake, including a family assessment that identifies areas of needed support and growth opportunities
- Complete Youth Assessment Screening Instrument (YASI) Assessments and Case Plans
- Develop individualized Service Plans and review and revise plans within designated timeframes
- Tailor evidence-based services to meet the needs of the youth and their family
- Provide mentoring, family counseling, individual skill building, coordination and/or referral to prosocial activities, and assistance in obtaining employment
- Co-facilitate evidence-based groups on topics including healthy decision-making, life skills, anger management, and communication
- Connect youth and families to community resources
- Administer satisfaction surveys at Service Plan reviews
- Complete case note documentation in a timely manner and provide monthly progress summaries
- Participate in meetings with school staff, Social Workers, and other providers and schedule staffings as needed
- Accurately document and track required data in accordance with established program policies and procedures
- Complete Discharge Summary outlining the youth’s progress towards achieving goals in the Service Plan, improvement in functioning in areas such as employment, school, substance use, self-esteem, and mental health symptoms, and recommendations for follow-up care
- Other duties as assigned
Competencies and Skills:
- Knowledge of applicable social service and community service providers in Sawyer County, preferred
- Strong oral and written communication skills
- Effective interpersonal skills in bringing people together to solve problems
- Working knowledge of Microsoft Office (including Outlook, Word, and Excel) & web-enabled databases
- High level of skill in organization, attention to detail, accuracy, and the ability to handle multiple tasks
- Understanding of and experience providing services using person-centered, Motivational Interviewing, and Trauma-Informed Care
- Exercise good judgment
- Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities
Qualifications:
- Degree in human services field, preferred
- At least two (2) years of experience working with youth in the Mental Health &/or Substance Abuse field, preferred
- Pass a pre-employment background check
- Valid driver’s license and automobile with insurance
